How I get the information and why I have it
The personal information I process is provided to me directly by you for one of the following reasons:
Establish contact with you to explore and/or take up use of services from Dr Campbell Murdoch.
Book a clinical consultation and collection of your information that will support the safe and effective healthcare.
Be anonymised and used to support the development of health services provided by Dr Campbell Murdoch
Under the General Data Protection Regulation (GDPR), the lawful bases we rely on for processing this information are:
i. I have a contractual obligation.
Article 6(1)(b) GDPR; “processing is necessary for the performance of a contract to which the data subject is party or in order to take steps at the request of the data subject prior to entering into a contract”.
ii. I have a legitimate interest.
Article 6(1)(f) GDPR “processing is necessary for the purposes of the legitimate interests pursued by the controller or by a third party except where such interests are overridden by the interests or fundamental rights and freedoms of the data subject which require protection of personal data, in particular where the data subject is a child.”
What I do with the information I have
The information entered into the contact form is emailed to Dr Campbell Murdoch.
The information that you have provided is used to establish initial contact between you and Dr Campbell Murdoch, and to explore or use healthcare services offered by Dr Campbell Murdoch.
The information may be processed by an administrator/secretary who works directly with Dr Campbell Murdoch.
The information entered when registering and booking a consultation is held securely within Heydoc, a cloud based health record. Payment information is processed by Stripe.
How I store your information
Your information is securely stored within a private email account and/or in an encrypted computer database, stored locally and/or on the private secure cloud storage service.
The information you provide on the contact form, which is then sent via email to Dr Campbell Murdoch, is permanently deleted after 3 months if no onward use of service is established.
If you take up use of services provided by Dr Campbell Murdoch the information received from the contact, registration or booking form will be transferred into your health record in the Heydoc clinical system if it is deemed useful for your health care. Information that is not required for safe and effective healthcare will be permanently deleted.
Your data protection rights
Under data protection law, you have rights including:
Your right of access - You have the right to ask us for copies of your personal information.
Your right to rectification - You have the right to ask us to rectify information you think is inaccurate. You also have the right to ask us to complete information you think is incomplete.
Your right to erasure - You have the right to ask us to erase your personal information in certain circumstances.
Your right to restriction of processing - You have the right to ask us to restrict the processing of your information in certain circumstances.
Your right to object to processing - You have the the right to object to the processing of your personal data in certain circumstances.
Your right to data portability - You have the right to ask that we transfer the information you gave us to another organisation, or to you, in certain circumstances.
You are not required to pay any charge for exercising your rights. If you make a request, we have one month to respond to you.
